Having radar skill helps here, to see where the cops are.
To minimize encounters with the charging suv's (Rhinos), use the wide part of hwy 99, avoiding the tunnels, turning around at the offramp maze in the south central part of hwy 99, and turning around by cutting thorugh the golf course before the bridge, going to far side, then bear right onto hwy 99 (usually road block there), then right again back on to golf course and then left (east) back out the golf course (Rhinos sometimes will attack there so be careful), or bear right after golf course and return through left side of bridge (seems to be fewer Rhinos). In the image below the main idea is to stay on the wide part of hwy 99 between the green lines.
When headed west towards the offramp maze, bear right onto the off ramp, then follow it around to the north, go a bit down the straight, uturn left and then bear right again onto the offramp to head back east. This is almost never an issue at heat level 5, but at lower levels you may encounter cops on the return off ramp.
When headed west towards the offramp maze, if you exit on the left side of the off ramp, barely missing the water barrels, most or all of the cops will go straight there instead of following you. If you exit on the right side of the off ramp, most will follow you and some may take the left side off ramp in anticipation of you being there. If you have radar, watch the map to see if you were followed. If you weren't followed and there are no cops already on that off ramp, you can uturn on the off ramp and/or use that spot to evade.
Note the cops can't "see" your car if there is a hill or low wall between them and your car, so even if they have surrounded you on the other ramps, as long as they aren't on your ramp, you can remain still and eventually evade by waiting out the time (at heat level 5 it takes about 1 minute to cooldown with no evade skills (I didn't use any)).
I usually start pursuits near the offramp maze, then go to golf course, back to off ramps, to golf course again, then make a full speed right turn bouncing off the wall onto the sidewalk shown as the yellow line in the image. That sidewalk is narrow and above the main street below. I then bear left into the tiny alley and make a sharp left using a hand brake to the cooldown spot there, shown as the red dot on the image below. It's a one way jump so no cops can enter from in front, but may follow you from behind. It's a risky cooldown spot, with a large trash bin you want to turn before it or go around, and also watch the pursuit meter bar to see if you got followed; if it's not in evade mode, a cop is probably behind you so take off over the jump, then use the sidewalk (yellow line on image) on your right now, to avoid a road block that is usually setup on the main street below. Bear right on hwy 99, and then bear right onto the off ramp in the off ramp maze, and see if you were followed and try to evade there by waiting it out.
If cops follow you on that off ramp again, then continue the normal pattern, through the off ramp maze to head back east, and take a full speed left turn onto the other side walk opposite the one marked in yellow below and try the cooldown spot again. Sometimes just going around the off ramp maze will also get you away from the cops.
I made a training video showing the pattern I used. It's get's a little easier to anticipate what the cops will do after you've done several hundred pursuits. When taking out cop cars, I'd to imagine why I'm taking them out such as "that's the cop that made the busted meter fill so fast", "that's the cop that added traffic magnets to single player races", "that's the cop that blocked off the bus terminal ramps", ...
